Upperline Plus is available to Medicare and Medicare Advantage patients who are already established with an Upperline Health or Care Partner provider. Not a patient yet? Find an Upperline Health or Care Partner location below to get started as a patient first — you can ask about Upperline Plus once you’re established. Selecting a partner will take you to their website.
You can continue seeing your current doctors, including your primary care provider, while gaining access to additional support through Upperline Plus.
